Medical Withdrawal Request Procedure
- Complete an application for accommodations by visiting the department’s webpage. The department’s webpage may be found on the ÀÏ˾»ú¸£ÀûÍø’s website.
- Read and review the full Medical Withdrawal Policy.
- A personal letter from the student that details the reason for the medical withdrawal request
- A letter from a licensed professional that details the need for the medical withdrawal for the specific term.
- Upload supporting documentation with the application OR email / fax the documentation to the email address or fax number below
- Check your JagMail account for a follow-up email. It may take 2 weeks to receive a follow up message.
- After receiving a message via JagMail, schedule an accessibility via Navigate ÀÏ˾»ú¸£ÀûÍø OR visit staff during virtual office hours on Zoom. The dates, times, and Zoom links for virtual hours are listed on the homepage for the department.
- After meeting with departmental staff, during the accessibility meeting, you will receive more information regarding the next steps of the medical withdrawal request.
